(a) Surgical errors such as failing to conduct surgery with appropriate care and skill.
(b) Errors in diagnosing conditions.
(c) Prescription errors such as providing the wrong types of medication or prescribing an incorrect dose of medication.
(d) Test failures including incorrectly reporting test results.
(e) Errors during pregnancy including antenatal testing errors or failure to recommend antenatal testing for conditions such as Downs Syndrome.
(f) Errors throughout pregnancy and labour such as failure to monitor and treat gestational diabetes or respond appropriately to foetal distress.
(g) Failure to properly perform sterilisation procedures such as tubal ligation or a vasectomy.
